The Office of the National Coordinator for Health Information Technology (ONC) recently published this fact sheet about the Beacon Communities nationally.
Access the newest Community Health Needs Assessment. The study was conducted to identify the most important health issues in the state and by county, using a scientific method to validate health indicators and to compare results. The assessment identifies priority health issues, which will be best addressed by health providers working collaboratively. The Bangor Beacon Community hopes to improve some of these health indicators through its programs.
